Entry forms available at Children’s Fund office. All entries become the property of Children’s Fund. By accepting prize to be considered, winner agrees to release “Sponsors” together with respective employees, officers, licensees, licensors of any and all liability.


Eligibility: Employees, officers, directors and representatives of “Sponsors” and their immediate family members and those living in their household, as well as each person or entity connected with the prizes, production or administration of the contest are not eligible.


How to Enter: Hand-deliver a completed entry form/s to a Children’s Fund employee or representative or mail to Children’s Fund, 348 W. Hospitality Lane, #110, San Bernardino, CA 92408. Completed Entry Form is defined as an original form, containing your name, complete address, valid phone number, parent/guardian signature and email address if available.


Selection of Winners: Winners will be selected by a committee and will be notified by phone and/or email. Winners must respond within 72 hours from notification. Failure to respond within 72 hours may result in forfeiture of the prize and, in such case; “Sponsors” may select an alternate chosen entrant from among the remaining eligible entries.


Odds of Winning: The odds of winning depend on the total number of entries received.


Prizes: Two Grand Prize Winners will receive 10 tickets to an Inland Empire 66ers game on during the month of April at the San Manuel Stadium in San Bernardino; lead the parade with Mascot Bernie, throw out the 1st pitches of the game, and receive hot dogs, popcorn and fountain drinks for all 10 ticket holders; and books donated by First Book. Eighteen Runners-Up Winners will receive 10 tickets to an Inland Empire 66ers game in April 2015 at the San Manuel Stadium in San Bernardino; join the parade with Mascot Bernie, receive hot dog, popcorn and fountain drinks for runner-up only; and books donated by First Book. Children’s Fund reserves the right to impose certain restriction on use of the prizes. No substitute prize will be awarded. Anticipated value of the prizes is up to $200.


Conditions: Contest is subject to these official rules. By participating, entrants agree to be bound by these complete official rules and the decisions of “Sponsors” which shall be final and binding; waive a right to claim ambiguity in the contest of these official rules. Entrants shall agree to “Sponsors” use of the contest for publicity purposes in any media and to the use of the name, likeness, and hometown name and/or prize information of selected entrant as part of the publicity without any compensation or prior notice. Winners’ entries may be displayed at an April  2015 baseball game.


Limitations of Liability: By accepting the prize, winner agrees to release “Sponsors” together with the respective directors, employees, officers, licensees, licensors and agents illegible without limitation, and any person or entity associated with the production, judging, or administration of the Sweepstakes, from any and all liability, loss or damage arising from or in connection with awarding receipt and/or use or misuse of prize or participation in any prize related activities. Further, the Releasees shall not be liable for late, lost, delayed, stolen, misdirected, postage due, complete unreadable, inaccurate, garbled or unintelligible entries communications or affidavits, regardless of the method of transmission; telephone system, telephone or computer hardware software, and or other technical or computer malfunctions, lost connections, disconnections, delays or transmission errors; data corruption, theft, destruction, unauthorized access to or alteration of entry or other materials; any injuries, losses or damages of any kind caused by the prize; any printing, typographical, administrative or technological errors in any materials associate with the contest. “Sponsors” disclaim any liability for damage to any computer system resulting from participating in, or accessing or downloading information in connection with the contest and reserves the right to cancel, modify or suspend the contest should a virus bug, computer program, unauthorized intervention or other causes beyond “sponsors” control corrupt the administration, security or proper play of the contest.

Liability Agreement: By entering, entrant agrees to follow the official rules and hold harmless and indemnify “Sponsors,” and each of their respective parents, subsidiaries, affiliates, directors, officers, employees and agents against any and all liability damages or causes of action. “Sponsors” reserve the right to disqualify any person and void any entries of that person who it believes has tampered with the entry process or any other component of the contest, whose conduct or action affects the administration, security or fairness, integrity of proper conduct of the contest, or who fails to meet or abide by the contest rules and conditions.
